How can I support the shift to value-based care using MX Access?

  • Prepare daily action lists. Log in to MX Access and click on Encounters to identify if your patients have been recently admitted or discharged from an out-of-network hospital. (See the full instructions here.)

    • Track and manage your patient population using real-time hospital encounter data.

    • Provide fast, informed follow-up care after a hospitalization to reduce readmissions.

  • Identify high-risk patients. Log in to MX Access and click on Problems & Diagnoses to view a list of diagnosis descriptions. (See the full instructions here.)

    • Look for patterns in patient usage, such as the amount of times at an ED and reasons for each visit.

    • Search for worrisome patterns in patient lab values or other warning signs that your patient may need additional help.

  • Outreach proactively. Log in to MX Access and print the patient’s summary report. (See the full instructions here.)

    • Provide physicians and care managers with the latest longitudinal health records.

    • Contact recently discharged patients to coordinate follow-up care and appointments.

Why does this matter? Your value-based care program can’t succeed without fast, reliable, and quality access to data. A full 8 out of 10 ACOs cite integrating data from out-of-network providers as their top technology challenge. And half of ACOs have not been able to access behavioral health data or integrate with post-acute care and hospice facilities. Despite not having access to crucial data, 85 percent of ACOs have already invested in advanced analytics software. Access is proven to drive better results. One ACO saw a 9.6 percent increase in compliance over all reported ACO metrics through access to better data.
