How can I manage transitions of care using MX Access?

Viewing real-time patient longitudinal records, including discharge summaries, recent encounters, and labs with MX Access can help you manage transitions of care:

  • Review a patient’s discharge summary in real time. Log in to MX Access, click on Documents, then click on the discharge summary. (See the full instructions here.)

    • Review the discharge summary before the first post-hospitalization appointment.

  • Review patient admit and discharge history in real time. Log in to MX Access and click on Encounters to see a list of admissions and discharges. (See the full instructions here.)

    • Contact recently discharged patients to coordinate follow-up care and appointments.

    • Identify patients who need extra help and support, reducing the risk of readmission.

  • Review patient test results after being discharged from a hospital. Log in to MX Access and click on Labs to see a list of order items and test results. (See the full instructions here.)

    • Check to ensure the recommended tests have not been done to avoid expensive and time-consuming repeat tests.

Why does this matter? Fewer than half of primary care physicians say they receive the patient discharge information they need from hospitalists. Only 12–34 percent of discharge summaries reach aftercare providers by the time of the first post-hospitalization appointment, and most of those that do arrive contain inaccuracies. That leaves primary care providers unaware of 60 percent of pending test results returning after discharge, of which nearly 40 percent are considered actionable. The faster you learn about patient discharge, the faster your value-based care organization can follow up, and the higher your reimbursements. Post-discharge follow-up has been shown to create a savings of $4,845 per patient.
