Consolidate duplicate problems

As a user, I want repeat diagnoses to be condensed with the ability to expand so that the clutter factor is reduced, and I can see more diagnoses on the screen without having to scroll.

    2nd request by Dr. Bittman: Please consolidate diagnoses by description. User can expand and see which facilities sent that info and on what date
