Include encounter date in diagnoses tab

As a user, I want to see the encounter date listed in the diagnosis tab since the diagnosis date is not always populated, so I can easily identify when the patient had an encounter.

    2nd request: Brishay Terrell mentions discharge diagnoses, and problems & diagnosis dates are not being sent over

    3rd request: Ai mentions since messages rarely include diagnosis date, consider figuring out a different way to display the date
