Medicare ID in demographics

As a user, I want to see the patient's Medicare ID added to "More Demographics" so that all of the patients important information can be found in one location.

    Second request by Dr. Bittman @IEFMC:

    Consider flagging cohorts. ACO patients are critical for hospital or institutions to recognize the patients in risk-based contracts, etc. Hospitals would then know what to do with those patients.
