Admit Source

As a user, I want to see the admit source description listed instead of the code number in MX Notify so that I can quickly review where the patient was admitted instead of referring to a code.

Opt Out Patients

Duplicated home phone number

Under patient's demographics, the home phone number is duplicated (see attached). Please remove one of the duplicate phone numbers.
